Templateez exists because most professional intake forms on the market are either built by template companies that have never practiced, or pulled together from a CLE handout that was already five years out of date. We thought we could do better.

Who built this

Templateez is a product of AAA Designs, LLC, founded by a licensed attorney. Every form in the library is reviewed against current practice conventions, and the legal forms reflect the way the documents actually get used in litigation and transactional work — not how a template company guessed they should look.

The library started with the forms needed for the founder's own practice. When colleagues asked for copies, the same gap was visible across industries — healthcare practitioners with HIPAA forms missing required elements, trades contractors using generic service agreements that wouldn't survive a dispute, small businesses cobbling intake forms from Google search results. Templateez consolidates one well-designed library for all of them.

How we work

Every product on Templateez is a fillable PDF — fields you click and type, not a Word document you'd reformat. Forms are password-protected against editing (you can fill them, print them, and email them, but not change the structure). Two-page mockups let you preview the form before you buy.

Most products come as a two-document set: an internal intake form for your team, and a client questionnaire for the customer or patient to complete and sign. The two forms work together — the intake captures what you need to evaluate or quote the matter; the questionnaire captures the customer's signed authorization. Where it makes sense, we bundle the matched pair at a discount.

Bundles are organized by industry: a Legal Bundle for solo and small firm attorneys, a Healthcare Bundle for medical and allied practitioners, a Professional Services Bundle for consultants and financial advisors, a Trade Services Bundle for home services and contractors. Each bundle is 40–48% less than buying the same forms individually.

What we won't do

We're a template library. We are not a law firm and we don't have an attorney-client relationship with our customers. Our products are designed to be sound starting points — they save you the work of building from scratch — but you should review and adapt them for your specific practice, jurisdiction, and risk profile. If you're working in a state with unusual rules (community-property jurisdictions for real estate, specific consumer-protection rules for trades, state-mandated privacy disclosures for healthcare), a final review by counsel licensed in your state is always worthwhile.

We also don't sell customer data. Every transaction goes through LemonSqueezy with PCI-compliant payment processing. We don't track browsing behavior beyond aggregate analytics. We never share customer information with third parties for marketing.
