Event Registration Forms: What to Capture Before the Day

Event registration is the first touchpoint between you and your attendee, and it sets expectations for the entire experience. A good registration form collects the data you need to run the event (head count, dietary restrictions, session choices) while also capturing the data you need to grow your business (company, role, how they heard about you). A bad one asks too many questions and loses registrations, or asks too few and creates chaos on the day of.

Attendee Information

Name, email, phone, company, job title, and — for multi-day events — whether they need hotel accommodations. For conferences: which sessions or tracks are they interested in? For ticketed events: ticket type (general admission, VIP, early bird, group rate). Our Event Registration form includes structured fields for all common event types.

Dietary and Accessibility

If the event includes food: dietary restrictions (vegetarian, vegan, gluten-free, kosher, halal, allergies). This is not optional — getting it wrong is a liability issue, not just a courtesy issue. Accessibility needs: wheelchair access, sign language interpreter, hearing loop, large-print materials, service animal accommodation. Ask these upfront so you can plan, not scramble.

Emergency and Liability

Emergency contact name and phone. For physical or outdoor events: medical conditions the organizer should be aware of, and a liability waiver (see our Liability Waiver form for the companion document). For events involving minors: parent or guardian consent and contact information.

Marketing and Follow-Up

How did they hear about the event? (Search, social, email, word of mouth, partner referral.) Consent to receive post-event follow-up communications. Interest in sponsorship opportunities or exhibitor space. These fields turn a registration form into a lead generation tool — every attendee is a potential future customer, speaker, or sponsor, and the registration form is your chance to identify which ones.
