Flooring Installation Intake Forms: What to Capture Before the First Plank
Flooring jobs go sideways when the estimator doesn't ask the right questions on the first call. Subfloor condition, material transitions, furniture handling, and moisture levels all affect pricing and scheduling. A flooring intake form that captures these details upfront eliminates the two most common problems: mid-job change orders and callbacks for squeaks and gaps.
Material and Scope
What's being installed: hardwood (solid or engineered), luxury vinyl plank (LVP), laminate, tile (ceramic, porcelain, natural stone), carpet, or epoxy coating? Which rooms? Total square footage? Existing Floor and Subfloor
What's on the floor now, and what's under it? Removing old flooring changes the labor estimate significantly. Carpet over plywood is simple. Tile over concrete requires a different approach than tile over plywood. If the subfloor is uneven, there's a leveling step. If there's asbestos-era tile, that's a hazmat consideration. Ask about the floor level relative to adjacent rooms — transitions between different flooring heights need trim pieces and threshold planning.
Moisture and Environment
Moisture kills hardwood installations. The intake should ask about the level (basement, ground floor, second floor), whether the home has a crawl space or slab, whether there have been any water issues (leaks, flooding, high humidity), and whether the rooms have consistent climate control. For slab installations, a moisture test is typically required before warranty coverage applies. Note this on the intake so it gets scheduled.
Logistics
Does furniture need to be moved? Is there stair work involved? Are baseboards being removed and reinstalled? What about appliances — will the customer disconnect the fridge or does the crew handle it? Stair installations are priced separately from flat-floor work. Furniture moving may be included or charged extra. The intake captures all of this so the estimate is complete.
