How detailed do my specifications need to be?

The more detail the better, but you don't need to be technical. Just describe your profession, what the form is for, what information you need to collect, and any specific fields, checkboxes, or sections you require. If you have a hand-drawn sketch or an existing form you want to improve, attach it to your email.

How many fields or pages can the form have?

Each option covers a standard 1–2 page form, which is typical for most intake and work order forms. If you need something significantly more complex (multiple pages, extensive tables, conditional fields), contact us before ordering and we can discuss a custom quote.

Can I get my logo or business name on the form?

Yes — custom branding (your logo, business name, colors, contact info applied to the top of every page of your custom form) is available as a +$19.99 add-on. Check the "Add custom branding" box at checkout. Branded files are delivered within 2 business days of receipt of your assets.

What if the form doesn't match what I asked for?

One round of revisions is included. If there's a genuine misunderstanding and we can't deliver what you described after revisions, we'll issue a full refund.

How long does it take?

2–3 business days from when we receive your complete specifications. If you need it faster, email us first — rush delivery may be available depending on current workload.

Can I order the same custom form for multiple staff members?

One purchase covers one business entity. You can print and use the form across your entire staff. You cannot resell or share the form with other businesses.
