You need marriage dates, separation dates, children's names and ages, custody preferences, asset inventories, debt schedules, income figures for support calculations, and often sensitive information about domestic violence or substance abuse. It follows the standard litigation format used across all 38 of our legal forms: page one covers case administration and client contact information, page two captures opposing party details and matter-specific checkboxes, and page three has key dates, court and stage tracking, a summary issues table, witness log, and progress notes.

What the Family Law Form Captures
